One of the aims of Chronic Disease Management is to introduce a schedule of preventative care with the simple objective of reducing the number of acute events and hospital admissions. Each admission is expensive, so significant savings can be made by addressing problems and managing patient welfare to the point where admissions are reduced.

The Hippocrates product developed by Defacto using Microsoft tools manages the nurses’ diaries and audits patient progress and provides valuable analysis in report and immediate on-screen format.
